For NHS student-athletes

Are you interested in playing sports at Novato High School?
All athletes must register, submit a physical form signed by a doctor, and take a concussion test every year. No student-athlete may participate with any team without registering prior to the first day of practice, submitting a physical form, and taking a concussion test. Once registered you will receive an email to set up an appointment to take a concussion test (beginning in July).

REGISTER FOR ALL SPORTS YOU INTEND TO PLAY FOR THE YEAR BY CLICKING BELOW

Step 2
Complete this Physician's Clearance Form
dated AFTER MAY 2025.
Must have form signed to complete registration

Step 3
Register to participate in sports by going to step 1 and then take an impact concussion test.
Required once per academic year.
NO SIGN UP NECESSARY but You must complete Step #1 in order to take test. An email will be sent to you in July with dates to take the test.
